Full Referrals Policy
Dr. Johnson is a participating provider in BCBS of ND and other commercial
health care plans including TriCare and TriCare for Life. Most plans
covered under ND BCBS include mental health services. Generally, subscribers
are allowed 30 mental health visits in a calendar year beginning January
1 and ending December 31. Questions about specific plan provisions
should be directed to ND BCBS customer service.
In general, most TriCare subscribers may schedule an appointment without
a referral. Specific questions can be answered by our staff or by contacting
TriCare customer services.
Self
Persons seeking services at the Center do not need a physician or other
healthcare provider in order to schedule an appointment.
Healthcare provider referral
We welcome the referrals of patients from other health care providers
in our area. Providers with questions about referring to our office may
contact

Network referral
Some BCBS subscribers may require an out of network
referral in order to continue to receive services at the Center at “in network” rates.
In network generally means the subscriber can continue at their plan’s
80/20 or 90/10 rates. BCBS policy for out of network is generally lower
and dependent on each plan. Patients may request their provider to request
an “out of network” referral for them. For more information

Employee Assistance Plans
Services from the Center may be included as part of some EAP plans.
Questions about receiving services as part of EAP can be directed to
our office.
Specific questions may need to be directed to Human Resources at individual
companies.
