Participating Provider
Dr. Johnson is a participating provider for many health insurance plans.
We file insurance claims for our patients as part of our customer service.
Some insurance plans require co-pays at the time of service. If co-pays
are part of your plan, you will be expected to pay this at each visit.
You will be asked for your insurance card information at the time of your
first visit and periodically after that so we can update our records for
any changes.
We submit our billing charges electronically to most of our insurance payers.
Because we do this daily in our office, we are able to provide you in most
cases with current information about reimbursement from your insurance
company to us. This means you can know as soon as we know, in most cases.
In Network/Out of Network referrals
Many health insurance plans, including BCBS of ND require subscribers to
enroll in provider networks such as Trinity Select Choice and others. For
mental health visits under ND BCBS no referral is required for the first
five (5) visits, regardless of network. After the first five visits, a
request for out of network service will be sent to the director of the
plan. Generally, the request for out of network services is granted. If
your health care provider referred you to the Center and Dr. Johnson, we
will ask for a letter of referral.
TriCare
Dr. Johnson has been a participating provider of mental health services
to TriCare members and their families for over 15 years. No referral is
required from the PCM for mental health visits if you are a dependent.
If you are active duty military, you must follow the procedures for referral
to non-military providers.
TriCare dependents are eligible for eight (8) sessions in mental health
before TriCare requires an update and request for additional appointments.
We are able to complete requests for additional sessions electronically
and generally receive the additional sessions requested.
Other plans
Other insurance plans we accept include United Health, Cigna, and BCBS
of MN among others. If you have a question about your health plan coverage
for seeing Dr. Johnson, please call or contact us. We will do our best
to answer your questions and in some cases determine your eligibility to
have services in our office.
Statements
Statements are sent out by the 5th of each month. The policy of the Center
is to require full payment of any amount due each month, less the amount
filed for your insurance and co-payments. Partial payments may be arranged
for some patients; however, this is individual and must be arranged through
the office manager.
We accept Visa and MasterCard as credit card payments as a convenience
to those patients who prefer to pay using this service. We do charge interest
on all unpaid accounts after 30 days.
